hold light/rev counter

hold light/rev counter
Hi all I could really do with some help with a little problem I have.
Most of the time my Bongo runs really well with no issues then for no reason the rev counter goes all wibbly wobbly the hold light starts flashing and the gearbox (auto) drops down i cog and wont change back up, I stop and turn the ignition off leave it a couple of minutes restart and normally the rev counter goes back to normal and the hold light goes out and we carry on our merry way very occasionally i need to stop and start it a couple of times before the rev counter goes back to normal.
So where do I start looking to solve the issue.

Jumping onboard here (hope you don't mind) as I have very similar transmission problems and need to read the flashing hold light sequence. Just looking at the link above, it seems a jump lead is necessary. Can anyone tell me where do I get one of these from?

Paper clip from the stationery store Any bit of wire to jump between the two connections shown, short length of wire from a bit of mains flex, paper clip whatever you have to hand!

Mines a diesel, automatic 4wd. Getting two codes from the hold light sequence:
12 - throttle position sensor
16 - not listed in the Automatic Transmission Malfunction Codes (from the philmcfadden link above), but in the Engine Malfunction Codes its described as EGR position sensor.
Can anyone advise, if it's possible to remedy these faults easily, or will the van need to be taken to a specialist?

Given that the rev display fails I would check the RPM sensor first, mobile at present but a search will locate its position on the diesel.
