Problem when pulling away when warm and with load on board

Recently I have noticed that my Bongo has started juddering when pulling away from a standstill, (I have a 2wd 2.5D auto) I have only noticed this when it has been driven for a while so it is warm and normally when we have all the kids on board (I have 3) and other bits and pieces, it only happens for a couple of seconds and then is smooth up through the gears and back down , if I accelerate whilst moving slowly like you quite often do on a roundabout I don't get the problem it only happens when I move from stationary.
