I wouldn't wish to alarm you; but I had similar symptoms (with coolant pouring out the header tank), and it ended up being a cracked head! Now, with a new one installed, the van runs very well, and the coolant level is rock solid!
I hope it's not the head! It may well not be!

Is the likely cause the water pump seal?

What green coolant do people use?
Have you had to top of coolant over this period? You could try some u/v dye and a u/v torch to see if it reveals any pinhole leaking or weeping :roll:
